Reset your Online Form Portal Multi-Factor Authentication method
MFA methods can be updated anytime by following the below steps.
- Login to Online Form Portal.
- Select Settings.
- Select Reset Multi-Factor Authentication.
- Resetting your MFA will log you out. Upon your next login, you will be required to setup a new MFA method.
- Select "Reset MFA" to continue.
- Select "Yes" to confirm reset.
- You will be logged out of Online Form Portal.
- Log back into Online Form Portal
- After entering your password you will be prompted to setup your MFA.
- Choose Phone or Authenticator App. and Continue.
- Follow the prompts to reset your MFA.
- Once the MFA has been reset you will be logged into Online Form Portal.
Note: If your account has a recovery email, MFA will be reset for all email addresses on the account.
